We want to make it as easy as possible for you on Sunday mornings! If you're new, we need to collect some basic information for your child in order to ensure their safety in our kids environments. Below is a form that will get you started (it will ensure that your child is in our system, ready to check-in). We will have an additional form for you to fill out at GVC that contains much more sensitive information (like D.O.B., Grade, Schools Attended, etc.). We want to keep that information as secure as possible.

If you're filling this form out after 8PM on the Saturday prior to your first visit, we cannot guarantee that your information will be in our system, so please plan on coming early to fill out the proper forms.
